Physical Therapy
Do I need a referral from my doctor?
No. You can book an evaluation directly — no referral, no gatekeeping, no waiting on someone else’s paperwork. Bring one if you have one and it is useful context, but nothing is held up without it.
How long is a session?
Sixty minutes, one-on-one, start to finish. Not thirty minutes, and not handed off to an aide partway through. The full hour is the whole point of the cash-pay model — it is what insurance reimbursement makes impossible for most clinics.
How many visits will I need?
A typical plan of care runs 8–12 visits. You get an estimate at your evaluation, before you commit to anything, and it is revisited as you progress. If you need fewer, you finish sooner.
Who will I actually see?
A Doctor of Physical Therapy, and the same therapist every visit. Continuity is deliberate: the person who assessed you is the person who progresses you, so nothing gets re-explained and nothing gets lost between appointments.
How do I book an evaluation, and what does it cost?
An evaluation is $200 for a full 60 minutes and you can book it yourself — no referral, no phone tag. New patients also fill in the intake form before the first visit so the hour is spent on you rather than on paperwork. Already a patient? Book a follow-up session directly. Not sure yet — take the free 20-minute call first, or call the clinic on (320) 433-8361.
Do you offer virtual physical therapy?
Yes, anywhere in the world. You work with a Doctor of Physical Therapy who assesses you over video and builds the full follow-along program — every exercise, every progression, demonstrated and scheduled — delivered through our Custom App so you always know what today’s session is. Start with the 45-minute virtual consult on Google Meet.
What is Complete Care?
Unlimited physical therapy for a flat monthly price instead of a per-visit one. Complete Care on its own is $599 a month with no gym membership. For $648 a month you get Complete Care at the discounted $499 plus a Skilled Signature membership at $149 — so for $49 more than unlimited therapy alone, the entire gym comes with it: 24/7 access, CrossFit, tanning and red light therapy included.
It is built for the person who is not looking at four visits and done — a long rehab, a nagging problem that keeps coming back, or somebody who wants a Doctor of Physical Therapy on hand while they train hard. Ask about it on the free 20-minute call.
What should I bring to my first visit?
Answer needed — clothing, any imaging or prior records, and how early to arrive.
What kinds of problems do you treat?
Answer needed — the scope you want stated publicly, and what you refer out.
Gym Membership
Where are you located?
133 Sandberg Rd, Monticello, MN 55362 — one location, and everything is in it. The building has served this community for roughly thirty years and is one of the largest training facilities in town. Front desk: (763) 295-3002. Want to walk it first? Book a tour — no pitch, just the space.
What are your hours right now?
5am to 9pm Monday through Saturday, and 7am to 2pm on Sunday. That changes at the end of the month when 24/7 member access goes live.
Is the gym really open 24/7?
It is about to be. 24/7 member entry goes live at the end of the month and runs through the members app — your phone is your key, so you train at 4am or 11pm without waiting for someone to unlock the door. Until it switches on, the hours above apply.
What’s actually in the building?
Six ways to train, all open now:
- Full gym — machines and free weights
- Full cardio room
- Turf room for sports and field work
- CrossFit — classes start in October
- Studio gym — classes available
- Boxing gym — classes available
A performance room and a PT and recovery room are coming next. Physical therapy and performance testing happen in the same building, which is why a rehab program here can use real equipment instead of a resistance band in a side room.
What does a membership cost?
Every price is on this page. No quote, no form, no phone call to find out.
- Skilled Foundation — $79/mo. 24/7 access through the members app, every gym area, locker room.
- Skilled Signature — $149/mo. Everything in Foundation plus unlimited CrossFit, unlimited tanning and unlimited red light therapy.
- Skilled Foundation, Student — $55/mo. High school and college.
- Household Foundation — $226/mo. One Foundation membership plus up to three people in the same household.
- Household Signature — $356/mo. One Signature membership plus up to three people in the same household.
There is a low $20 initiation fee on every tier, and it is not overhead — it goes into a pot that buys new equipment for the floor. A member can be on the household rate or the student rate, not both. Sign up here.
Do tanning and red light therapy cost extra?
No, and they are not sold separately either. Both are unlimited and included with a Skilled Signature membership at $149 a month, along with unlimited CrossFit. That is the whole reason Signature exists.
Can I take a class?
Yes. Studio and boxing classes run out of their own rooms and are private-pay, booked with the trainer who runs them. CrossFit classes start in October and are unlimited with a Skilled Signature membership.
Class times are not published yet — the studio and boxing schedules come from the trainers who run them, and CrossFit signup will open in Club Systems in October. Call the front desk on (763) 295-3002 and we will tell you what is running this week.
Is there a contract or a minimum commitment?
Answer needed — month-to-month or a term, and any minimum commitment.
How do I cancel or pause my membership?
Answer needed — notice period, how to submit it, and whether a freeze is available.
Can I bring a guest?
Yes. A guest pass is $20. It is the same thing as a day pass — one person, full run of the gym for the day. Pick it up at the front desk.
Is there a minimum age to join?
Answer needed — minimum age, and whether it differs for unstaffed hours.
Do you sell day passes, and what do they cost?
$20 for the day. Day pass and guest pass are the same pass at the same price — it gets you the full gym for that day. If you are in town for a week it is worth comparing against Skilled Foundation at $79 a month.
Performance Training
What does performance testing measure?
Testing runs on VALD ForceDecks. It measures power output on both the concentric and eccentric side of a movement, vertical jump, inter-limb comparison — how much each leg is actually contributing — and isometric pull. The output is a number you can retest against, not an opinion.
What does performance training cost?
$250 a month for group performance training, three days a week. In person or virtual, same program, same price. Sign up here — payment is built into the form.
Can performance coaching be done remotely?
Yes, anywhere in the world, at the same $250 a month. Your program is written for you and delivered through our Custom App, so the session on your phone in another time zone is the same session running on the floor here.
If you are training around an injury or coming back from pain, the program moves over to the physical therapy side and a Doctor of Physical Therapy takes it — that is not a trainer’s job.
Do I have to be an athlete to be tested?
Answer needed — who testing is open to, and any age or training-history requirement.
How do I book a performance test, and what does it cost?
$100, and you can book it on its own calendar. That covers a 30-minute test now plus 15-minute retests at six weeks and twelve weeks, so you are buying the comparison, not a single number on a screen.
Is nutrition guidance included, or is it separate?
Separate, and priced either way. Nutrition coaching on its own is $200 a month. Added to a $250 performance training package it is $150 a month.
Billing & Policies
Do you take my insurance?
No. Skilled Fitness & Therapy is cash-pay. You pay for the session, you know the price before you book, and there is no claim, no deductible math, and no bill arriving three months later.
Why don’t you take insurance?
Because insurance decides the length of your visit, the number of visits you get, and what counts as done — and none of those decisions are made by the person treating you. Staying out of it is what makes a full 60-minute one-on-one hour possible. You get the care you need rather than the care a policy authorizes.
What does everything cost?
All of it, in one place. There is nothing worse than hunting for a price or handing over your details to get a quote.
- Physical therapy evaluation — $200, 60 minutes. Treatment packages are built per client after that.
- Complete Care — $599/mo for unlimited physical therapy, or $648/mo with a Skilled Signature membership included.
- Memberships — $55 to $356/mo depending on tier, plus a low $20 initiation fee.
- Day pass or guest pass — $20.
- Group performance training — $250/mo, three days a week, in person or virtual.
- Nutrition coaching — $200/mo, or $150/mo added to a training package.
- Performance testing — $100, including the retests at six and twelve weeks.
Can I use my HSA or FSA?
For in-person physical therapy here in Monticello, yes — HSA and FSA cards work, and we provide a superbill you can submit to your plan yourself. Whether it comes back is between you and your plan.
Virtual clients outside Minnesota are cash-pay with no reimbursement, so treat the price you see as the price you pay.
What happens if I need to cancel or reschedule?
Answer needed — the notice window, any fee, and how to reschedule.
Do you offer packages or payment plans?
Yes. Your treatment package is built around what you actually need after the evaluation, and you pick how to pay for it:
- In full — and you take 15% off.
- Split into two or three payments.
- Weekly, for a slight increase.
You see the number before you agree to anything.
How do I reach an actual person?
Front desk, gym and anything general: (763) 295-3002. Physical therapy and anything clinical: (320) 433-8361. Email info@skilledfitnesstherapy.com, or send an appointment request and we come back to you.
